WHAT ARE THE QUALIFYING CONDITIONS FOR CANNABIS IN UTAH?

We have made a list on the front home page of our website to refer to. The "more FAQ's" button at the bottom of this page will link you to the states website of qualifying conditions. If you need clarification on if a condition qualifies please contact our office.

IS THE RECOMMENDATION LETTER THE SAME AS A MEDICAL CANNABIS CARD?

No. A cannabis recommendation letter will allow you to get products from a dispensary until the end of 2020. Patients need to apply for their medical card through the state in order to get cannabis. You can apply now and we recommend applying as soon as possible.

HOW DO I APPLY FOR A MEDICAL CARD IN UTAH?

The process is pretty simple and is laid out in three steps. You will need to create what's called a UtahID account and put your information into what's called the electronic verification system (EVS). This is the state's website to verify cannabis patients. You can access these steps through our website by clicking the "more" tab on our website then clicking "how do I apply for my medical card." We will walk you through and complete this application process as part of your first appointment in our clinic.

WHEN DO I NEED TO RENEW MY CARD ONCE I GET IT?

Once your card is issued you will need to renew it within 90 days. To do this you will need a follow up visit in our clinic. After the 90 day follow up visit, the state will then set your card to expire every 6 months by default. You will renew your card through the same website and with the same profile as the card application.

WHEN WILL I GET MY CARD?

Shortly after we approve your application through our clinic with one of our qualified medical providers (QMP), the state will email you your card.
